• Delegate Change Form

  • Use this form before Sept. 20, 2024, if your church needs to make changes to certified delegates or alternates.

  • If a certified alternate will be replacing a certified delegate at convention, there is no need to submit a form, but please let Kathy Traynor know in advance via email.
